چکیده

Background and Purpose—Atrial fibrillation (AF) is an independent risk factor for stroke. Recent studies have demonstrated that the CHA2DS2-VASc scheme is useful for selecting patients who are truly at low risk. The goal of the present study was to compare the risk of ischemic stroke among AF patients with a CHA2DS2-VASc score of 0 (male) or 1 (female) with those without AF. Methods—The study enrolled 509 males (CHA2DS2-VASc score 0) and 320 females (CHA2DS2-VASc score 1) with AF who did not receive any antithrombotic therapy. Patients were selected from the National Health Insurance Research Database in Taiwan. For each study patient, 10 age-matched and sex-matched subjects without AF and without any comorbidity from the CHA2DS2-VASc scheme were selected as controls. The clinical end point was the occurrence of ischemic stroke. Results—During a follow-up of 57.4 35.7 months, 128 patients (1.4%) experienced ischemic stroke. The event rate did not differ between groups with and without AF for male patients (1.6% vs 1.6%; P 0.920). In contrast, AF was a significant risk factor for ischemic stroke among females (hazard ratio, 7.77), with event rates of 4.4% and 0.7% for female patients with and without AF (P 0.001). Conclusions—AF males with a CHA2DS2-VASc score of 0 were at true low risk for stroke, which was similar to that of non-AF patients. However, AF females with a score of 1 were still at higher risk for ischemic events than non-AF patients. (Stroke. 2012;43:00-00.)

Evaluation of transesophageal echocardiography in detecting cardiac sources of emboli in ischemic stroke patients

Background: Embolus is one of the causes of ischemic stroke that can be due to cardiac sources such as valvular heart diseases and atrial fibrillation and atheroma of the aorta. Transesophageal echocardiography (TEE) is superior in identifying potential cardiac sources of emboli. Due to insufficient data on TEE findings in ischemic stroke in Iran, the present study was done to evaluate TEE in...

The Prevalence of Prescription of New Anticoagulant Drugs and Its Determinant Factors in Patients With Ischemic Stroke and Non-valvular Atrial Fibrillation

Background: Patients with ischemic stroke and underlying Atrial Fibrillation (AF) have a high risk of recurrent embolic events. New Oral Anticoagulant (NOAC) is highly effective and reduces the risk of recurrence in AF-associated Ischemic Stroke (AFAIS). Objectives: This study aimed to determine the prescription pattern of NOAC and its determinant factors in patients with non-valvular AFAIS. ...

Transcranial Doppler application for risk assessment of stroke in patients with mechanical mitral valve

Introduction: Observing microembolic signals in brain vessels by transcranial Doppler technology is a very common finding in patients with mechanical heart valve. These signals are almost always clinically asymptomatic. The aim of this study was assessment of transcranial Doppler in detecting probability of stroke in patients with mechanical heart valve. Methods: This descriptive study was ca...

Cardiovascular complications and acute ischemic stroke after the treatment with epinephrine in an anaphylactic patient

Anaphylaxis is a serious clinical condition that can affect all ages and many systems (skin, gastrointestinal system, respiratory and cardiovascular systems). There are a few case reports of cardiac and cerebrovascular complications due to the anaphylaxis and/or the treatment with epinephrine. A 69-year-old female patient with known coronary artery disease (CAD), coronary artery bypass graft (C...

Can patients at elevated risk of stroke treated with anticoagulants be further risk stratified?

BACKGROUND AND PURPOSE Patients with atrial fibrillation have a varied risk of stroke, depending on age and comorbid conditions. The objective of this study was to assess the predictive value of stroke risk classification schemes and to identify patients with atrial fibrillation who are at substantial risk of stroke despite optimal anticoagulant therapy. METHODS Seven recognized classificatio...

Transient ischemic attacks in patients with atrial fibrillation: implications for secondary prevention: the European Atrial Fibrillation Trial and Stroke Prevention in Atrial Fibrillation III trial.

BACKGROUND AND PURPOSE Transient ischemic attacks (TIAs) are infrequent in patients with atrial fibrillation, and little is known about the long-term prognosis and response to antithrombotic therapy. METHODS This study was a pooled analysis of participants in 2 randomized trials, the European Atrial Fibrillation Trial and the Stroke Prevention in Atrial Fibrillation III Trial, comparing those...
